In consideration of the fee paid, the participant understands that there is a risk of personal injury in the course of instruction and, with this knowledge, agrees to assume the risk of any injury and damages to participants during training. 

Specifically, the participant agrees to hold harmless the School and all other individuals, organizations, sponsors, promoters, operators, hosts, instructors, associations, schools, owners, officials, directors, employees, and other participants connected with the event from all losses, damages, injuries, causes of actions, claims, or complaints in the event that the participant is damaged or injured in any way during the participation, instruction and/or performance of any exercise or during any activity associated with the event location or during transit to or from the event.

Participant further agrees to strictly obey instructors and observe safety rules.

Because of the physical demands of this instruction, the participant understands that he/she must be in good physical condition to participate in the event. Participant understands that in case of injury, the only medical treatment Balance Jiu Jitsu Ltd will provide is first aid.

Participant agrees that any pictures, audio, or visual recordings taken of him/her in connection with Balance Jiu Jitsu can be used for publication, promotion, articles, shows, and advertisement without additional consent and without compensation at this time or any other time.

Regarding COVID-19: 

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. 
I further acknowledge that Balance Jiu-Jitsu LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that Balance Jiu-Jitsu LLC can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, club staff, and other club clients and their families.

I voluntarily seek services provided by Balance Jiu-Jitsu LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
 

I attest that:

•  I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

• I have not traveled internationally within the last 14 days.

• I have not traveled to a highly impacted area within Canada in the last 14 days.

• I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. 

• I have not been diagnosed with Coronavirus/Covid-19 and have not yet been cleared as noncontagious by state or local public health authorities.

• I am following all CDC-recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

• I hereby release and agree to hold Balance Jiu-Jitsu Ltd harmless from and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the club, or that may otherwise arise in any way in connection with any services received from Balance Jiu Jitsu Ltd.  I understand that this release discharges Balance Jiu Jitsu Ltd. from any liability or claim that I, my heirs, or any personal representatives may have against the club with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Balance Jiu Jitsu Ltd. This liability waiver and release extends to the club together with all owners, partners, and employees.

• I have read and understood this release and agreement and agree to its provisions. I am not under the influence of any drugs, alcohol, or other intoxicants. I am not suffering from any illness or incapacity. I am over 18 years of age. (If not over 18 years of age, parent or guardian must sign.
